Survey for upcoming Plastic Surgery Seminar in Raleigh NC

There has been some interest in having a plastic surgeon talk with our group about plastic surgery options available to us. Dr. Schwarz in Raleigh NC has graciously agreed to do a talk for our group and has asked if we have questions, concerns or issues that we might like to have him address.

I have put together a brief survey to try to collect some information in preparation for this talk. If you are interested in finding out about plastic surgery options, costs, etc... please take a few minutes to fill out the survery.
