Re: scheduling the surgery ... That really depends on the office, I know that where my husband and I had our surgeries the wait is usually longer, but I also know that insurance usually requires that you have met with your surgeon within 30 days of surgery, so it might be just on the horizon.
Here's a list of questions that I had for my appointment. I hope you find some of them helpful:
