DENIED!!!

I just called my insurance provider (State Health Plan PPO) and found out I've been denied.  Everything was sent in July 17th.  Some more documentation was sent in that they requested on Aug. 15.  I've called every week and today they say the computer shows I was denied. 
My BMI is 39.  I have type 2 diabetes taking 2 shots and a pill everyday.  I'm on 2 BP medications and cholesterol meds.
No I haven't been overweight all my life, only for the last 5-6 years.  I have really been struggling with this weight and have tried everything.  I feel like I am slowly dying.  I just don't understand.

First find out what the appeal process is. Many people have appealed and got approval.  Also if you are not going to a Center of Surgery of Excellence this might help also
