Federal BCBC Questions?

Besides having to be above 40 BMI, what else do they require?

Unfortunately, I had to delete this post due to privacy concerns.

Got a lap band in 2008. Tried hard, but didn't lose much weight & developed swallowing problems. Fought my insurance company for almost a year & finally had a band to DS revision on 5/11/12. Have now lost 125 pounds. Yay!

I have Federal BCBS.  It was VERY easy to be approved.  The benefits are listed as a BMI of 35 with two comorbitities or a BMI over 40.  Mine was 47.  My doctors office said that they needed proof of my BMI being over 40 for 5 years.  I did have other issues but according to the insurance company as long as I was over 40 it was medically needed. I agree.. call the service Number for BCNS. My BMI was 49 plus Dr. wanted weight record for 5 years. But I don't know if that is what he needed or the BCBS ins. Mine was approved very quick once summited. Good Luck Myrtis
