Pam and Shelli have given you good advice. Pam is correct, hyperlipidemia or high cholesterol is considered a comorbid condition. You probably will have to have a sleep study done as well, if you are a snorer and/or if you wake with headaches, there is a strong possibility that you might have sleep apnea another comorbid condition, but honestly, it only takes one to get you in with a BMI of 35 or more.
RE your insurance, call them and ask them point blank with there coverage is for WLS surgery with your particular policy, while BCBS generally covers it, your employer may have put special 'exclusions' into your policy. Beyond that, you want to know what they need in terms of supporting documentation. Do they require documentation of a supervised diet? How many months do they require? How long ago ... i.e., within the last year? 5 years? Do they require a 5 year weight history? etc...
