Sorry for the delay in getting back to you with the information you need - it has been a crazy week!
I think by this point, I would call your insurance company and get a status report. If for some reason they have not received everything they need, I would certainly call Connie at 704-355-6990. She coordinates all of the submissions, so she will oversee that it gets done. She is VERY sweet and will help in any way she can. Just a warning, I recently had to call her about something and it took a couple of days for her to get back with me - she is extremely busy because they are preparing to move offices soon.
