Update...
Wendy everyone is right normally you can correct this situation I know it is a real pain.
I was denied twice for silly things such as my phych was older than 15 months ok no problem had another one .
Second denial was Lee and I had not met with surgeon to discuss surgery complications etc..The doctors office had to explain to bcbs that I would meet with the surgeon after I had been approved.
I wrote an appeal letter and hand delivered it and got approved in two weeks.
I will be glad to send you a copy and paste of my appeal letter if you think it would help you.
Everyone told me insurance plays a money game well I am not sure about that but they sure seem to make it really tough.
Dont take it personally.
It is your doctors job to get this for you but sometimes like I had to take it into my own hands and had to stay on top of the insurance and the doctors.
I was even told by my surgeons office they never had such a hard time as they did getting my insurance to approve.
Hang in there you can do this.

Sorry I wasn't more specific when I made the post just too tired (from the new job) and pissed at the Dr's office! I had told my Dr when I had my pre-op that if I get denied "I plan to appeal!" That goes without a question! I'm a very determined person, this set back will only make me come out fighting harder! I haven't gotten an answer yet from anyone about "why" I was denied. I'll be sure to let you all know when I find out!
