Who told you that you need to be cleared by a social worker? Do you have a family Dr? It was my family Dr. that referred me to the bariatric surgeon and not a social worker. The only time I ever met a social worker or psychologist was after I was approved for surgery by the surgeon and this was during the pre-op clinic session.

runfatgirlrun is right, first step is get your family Dr refer you, then you will follow a process with the bariatrique team where they do have a social worker you will see before surgery.
