Help with Insurance...Please.....

I currently have BCBS of MT, Blue Choice....on the list of insurers for MT this one seems iffy...some are approved, some not......As we all need approval....I am considering switching to New West in open enrollment next month, there's only one entry for that insurance and it was an approval. Is there anyone out there that has experience and can help me???? I work for the State and I have heard that they do not ever ok WLS!! Does anyone know how to go about changing this? Does OH have a pre-made form to send in to the INS. companies?? Thanks in advance for any and all advice!! Diane
