Information on PHCS la.ms insurance PPO

Hope this finds everyone doing great. My question on the insurance is ,they require 5 years back with weights by doctors or such .Well I have more than 5 years back but not every year. Does anyone know if it has to be every year for 5 years back?Hope someone can help me.

You probably need to call and ask them directly.  I had PHCS and they flat denied me.  said it was not covered at all.  every insurance is different--and the underwriters are the ones with the answers. good luck! mary
