Military PCM letter..Proof Read..Please...
Tomorrow is my appointment with my PCM to get my consult to General Surgery. I'm a little worried about it.. I wrote a letter to map out everything for them. I would like you guys to proof read it for me and tell me if there is anything that should be changed. It's kind of long so thank you if you read all the way through.
These are the diets I have been on:
1. Low Fat
2. Grapefruit diet
3. Diabetes Diet
4. Adkins
5. Weigh****chers x2
6. Zone
This is my weight loss and gain story:
? Over the years my weight has continued to climb. I was never the small child thru school. In High School was the same way about 40lbs overweight. When I graduated I decided that I wanted to go into the military.
? I then started to diet. I was in the gym constantly and did the Low Fat diet. It worked then and I got down to 138, which was barely under my weight limit to get into the military. I joined the Air Force and did really well with keeping my weight down for close to two years, and then it started to climb again. I was over my limit then but I was still taping under the limit.
? I got the Depo-Provera shot for birth control without knowing it would make my weight problem worse. My weight shot up 35lbs putting me in the 170 range and I was not taping under.
? I went on the weight management program. I tried the Grapefruit diet that didn't work at all.
? My commander recommended the Diabetes Diet and that worked for a little while till I wasn't losing anymore and I failed the weigh in for the first time while on the program.
? So then I tried the Adkins which I did okay and lost some, but then I hit a plateau and failed the second time.
? I then started Weigh****chers, the points system was really easy to follow, but after about 3 months I hit the plateau again and failed for a third time.
? This time I was discharged out of the Air Force for my weight problems. I was two days short of 3 years & 175lbs.
? I stopped the dieting for about 8 months. Then my weight was around 210 and I really needed to do something I started the Zone diet about 2 months into I found out I was pregnant.
? I stopped dieting and then at almost 10 weeks I had a miscarriage.
? I didn't pay attention to my weight for a year, till I stepped on a scale at work and saw I was at 250.
? This time I started the Weigh****chers program and again I was doing really well. I got down to 225 and was holding steady.
? I found out I was pregnant again. This time it went full term and I had my son January 06'.
? After my son was born I was at 270 and I've managed since to lose about 15-20lbs doing the Low Fa****ching what I eat.
Now here I am at around 253. I've struggled for the longest time and I've been researching Gastric Bypass since last August of last year. I've been attending the local support group meetings to see if this is something I really want to do. I've decided it is. I want to use this tool to become a healthy person who can do anything with not having to worry about her weight all the time. To be able to make it up a flight of stairs without being out of breath at the top & stand for long periods of time without my knees & ankles hurting. To improve my self esteem to where I feel comfortable.
Thanks again for your imput and help. April*
Everything will be fine April, don't worry.. It sounds like you have made up your mind and all the pcm is going to do is refer you to general surgery. It is not as hard as one may think. I only had to mention it to my pcm and they referred me, then it was a fast course from there. Call me if you need to talk..
Rhonda
April you will do great at your appt. You have everything documented well. I had my appt with my pcm and 30 days later I had my surgery that was in July 05 before the storm. But you will still do great with general surgery. Dr Dolan is AWESOME!!!!!! We are all here for you to support you and help you through the process. Let us know how it goes!
