Yes we had the same problems....went to our PCP in Jan and he does not believe in the WLSurgery and he would not document the diets tried. I did an appeal with showing that even though the PCP did not state the weight loss I ask them to review the notes and see that weight loss was there. Also wrote about the problems that would come from being obese...ie; stroke, heart attack, or complications from trauma that obesity would cause if my husband did not have the surgery...believe it or not the oked after just one appeal with the letter I sent them. He meet all the required area except the 6mo MD supervised diet, but asking them to review the weight loss from one vist to the next in the appeal...they approved for surgery. So what they are looking for is that you can follow a diet and see a weight loss even though you dont keep it off. So if you have weigh****chers, LA weight loss or any type of documentation of weight loss it might work..good luck and let me know!
