Looking for contact name/number for Gulf Coast Medical

I am in the process of seeking a revision....having gastric bypass in Oct 2001. I am trying to locate a name/number for the program at Gulf Coast Medical. I understanding Live-lite did not renew their contract after Hurricane Katrina. My insurance company requires a letter showing a 6mo supervised weight loss diet. I am trying to obtain a letter to that effect. If anyone has information, please!! please!! please!! help me. Thanks
