I just did it. I faxed my appeal to the insurance company. I recently (August) was hired to be a speech implementor at a school district. When I received my insurance cards I called them inquiring about the surgery. They said there was a written restriction, but if I had medically documented proof that it was the only way, they would reconsider it and probably cover it. They told me I didn't even have to be denied because of the written exclusion I could go ahead and submit through appeals. Well, I finally received all of my proof (my family history of health problems from my grandparents doctors, a letter from the congregational nurse at my church, and my dr....whos letter said it was the only way for me to lose weight). I then wrote a 3 page letter myself explaining my life as a severe morbidly obese person in a "normal" sized world. I had some people I truly trust read over it and make any corrections in spelling/grammar usage and then faxed it. The insurance company said it would take 2 weeks for them to determine the appeal.... SO, please pray for me that they grant the appeal.
As soon as I hear something I will let you all know.... Just please, pray and agree with me that they grant me the appeal and allow me to have a new start at life.

I am hoping and praying with you. The surgery has already made a huge difference in my life. I was one of the lucky ones who didn't have to fight with the insurance company. Surely all the information you sent to them should convince them.
I'm sure two weeks seems like forever, but I think that is pretty standard. Good luck and keep us posted.
