Gathering Information for Surgery Consult

I need to gather all of my past medical history for my consult with Dr. Augustin, but apparently I need documentation from when my health problems began, which was when I was 8 months old....do they really want me to go back that far?? And if so, is it going to be hard to get that stuff?

It shouldnt be hard to do. You would have to call them to have them either fax you a consent form or have them mail it to you so they can send it directly to your doc. Hospitals and docs are required to keep all records so they should be able to dig em up for you.

Like Darlene said, it isn't hard, you just need to sign a consent form to release your records. The hard part is if all the information isn't at one doctor's office. Ask your mom, she would have the information on who to contact for what. Traci -93
