Policy info for Medicaid, specifically Health Care USA

 I am trying to find out what exactly is Healthcare USA's policy for weight loss surgery. Anyone have it in writing? 

I was told 6 months documented diet/attempted weight loss under PCP w/in 18 months of surgery request.

What they haven't told me is does it need to be once a month for 6 months, etc. etc...

Anyone have it somewhere that we can read it for ourselves? I got no help from the rep when I called.
