Mercy health plans through the state... Help

My husband works for the state and we have our ins through mercy. I have to do the 2 6 month weigth loss attempts. I have to see my dr once a month for six months. Do you remember what form it is he needs to fill out each month. I can't remember what it is called and my ins is very picky with that. Also does it matter what time of the month that I see him like every so many days or as long as it is every month. I also have to have 1 consultation with a dietician I think it is. When does this need to be done during the 6 months. My ins is being of no help with the detailed answers and my dr is so clueless when it comes to this. Any help is very app.

I also have Mercy Insurance. To my knowledge there was no form my doctor had to submit every month. If there was then my doctors office must have taken care of it on their own. I believe you have to go every 30 days. I did the medically supervised diet through Missouri Bariatrics and they had a class they did once a month. Who is your doctor? My doctors office took care of everything for me. Once they submitted it to Mercy it was approved the next day. 

I go to Dr Ramos my pcp. There is a certain form thay have to fill out to be submitted at the end. I just can't remeber what St Alexius called it. If it has to be exactly every thirty days my first month is shot. i went on the first of July and have been trying to get all this worked out since.
