Medicaid transportion
Sheila don't know if anybody else called for you, but I did and someone actually answered, even though it was after hours. Here is what she said:
They don't have email, but there is a number you can text your message to. She did n't know it, said needed to call medicaid to get it. The name of the company is Logisticare, they take reservation M_F 8-5. Need to schedule 3 business days in advance. However, she said if it was medically necessary that your dr. can call 7 am on monday and he will need to fax a form showing that it is urgent and fax it back.
They also need: Medicaid ID # (no medicare) and your medicaid needs to have transportation option, she said they usually do, but they check before the approve the transportation. They need address you are going to, name of Dr. and Facility you are going to. YOur name, dob, address to be picked up and reason for appt. They did say that someone else can call in for you.
If you need me to, I would be glad to have you email me the information and then I could call them for you. If you are going to see Dr. H. on Monday, maybe you could call Margie at home this weekend and ask them to help.
Anyway, hope this helps, and if I can help let me know. I wish I was closer to you. I have to go to see Dr. H on Tues for post op visit. If you could get to Collins (on HWY 13, north of Bolivar) and could wait until Tues. you are welcome to ride with me.
