PCP letter assistance needed

Can someone help me. My pcp letter is very basic and I'm afraid it's not enough for the approval. Can anyone send me a copy of a letter or an outline of one that I can bring to him to write properly. He isn't the greatest of doctors but he will do what i ask. I don't want to send in my paper work to get refused because his letter isn't good enough. Psch Dr said he would give me the go ahead. Which is great and everything else is ready to go. I'm walking on air! Thanks, Bel

Bel.....Did they not give you some example letters at your seminar????? I had one in my packet they gave me.....I gave them to my doctor to use as reference but I thought hers was kinda basick too....but they excepted mine......If you don't have the ones like I'm talking about then call the office and see if they can send you one out to give to your doctor for him to use....I think they mostly want to know if your doctor thinks you would benifit from having the surgery and all your medical problems that are related to being over weight. And probably how much weight you might have gained since seeing your doctor. If you don't get something worked out let me know....I know my doctor probably still has my example letters in my file and I will get you a copy of them if she does still have them....but I don't think the doctors office will care to send you one out if you ask...Good luck and let me know if you need a copy.....Take care....God Bless....Hugs...Tammy H.
