sample letter

Hey Guys! Whats up? I need one more thing before my consultation next tuesday...I hope someone here can help me. I need a sample letter from a nutrionist....my NP is going to write it, but she wanted some examples...I spent the four hours trying to find one with no results...does anyone have some examples? Please and thank you....I have a great feeling about this surgery...I know in my heart it will be approved..

Basically what they are looking for is a letter stating that you have a very good understanding that this is a "lifestyle change" and that you understand the nutritional needs for your new body and that you are enthusiasctic about making these changes and dedicated to making sure you stick with it and do not fail. She should make note of the general info you covered and state her confidence in your ability to put all this info to proper use and note that she is availabel to further assist you as a post-op. Also, a note as to how you've already implimented good choices in eating habits to already be on the right track as a pre-op would be helpful. Good luck!!! Tami
