Dr. Henry Buchwald Tribute Book
Dear Patient of Dr. Henry Buchwald—
Warm greetings! This is an invitation from Dr. Buchwald’s family for you to share in the preparation of a surprise project for him (a tribute book/scrapbook) in honor of his upcoming 80th birthday next summer, in 2012.
The project is a SURPRISE, so please do not wish Dr. Buchwald a happy birthday or mention that you know his birthday is coming up, if you should speak to him. ;-)
Do you have stories or anecdotes about Dr. Buchwald to share, good wishes, interesting memories, or thanks to give him? If so, we invite you to write, print, and/or paste up the gift of your own words and/or photos that will be compiled with those of other patients of Dr. B’s.
Here are some preliminary ideas of what you might include:
• How Dr. Buchwald changed your life (or your family’s or family member’s life)
• What you appreciate about Dr. Buchwald; your wishes for him
• A funny or telling story about Dr. B
• Before/after pictures or a photo of you with Dr. B
Please ensure that your page:
• Photocopies or prints well
• Covers ½ OR 1 full page • Has 1-inch margins and uses an 8½ x 11 inch format
• Arrives before March 20, 2012 if coming by mail (after March, please contact us by email or phone only)
Also, kindly attach a page with your name, phone, email and contact address.
