Paperwork packet is intimidating from surgeon

The paperwork is intimidating. If you don't blah blah blah quickly, your surgery/appt/seminar will have to be rescheduled etc etc. It sounds as if they want every xray ever taken of me. I don't have these things at my fingertips and we all know how slow doctor offices can be about forwarding records! 

There are some things I don't even remember WHERE I was when I had them done. I moved a lot one year. Do I call around or will it be okay without every medical record I have ever had? Such as dropping a bed rail on my foot and having it xrayed--it wasn't broken but every inch was bruised.

Also should I remember every diet I have ever been on? The exact dates? What I lost and gained and when?  I remember my first physician assisted diet at elementary age but that doctor was old THEN, probably not alive now.

Feeling nostalgic, going through old photos. It is helping me remember things I had blocked out. Seeing myself through the years--weight gain and loss.

Is there a free DS angel around? Would love to hear how things went pre-op from someone who has been through it.

First off, Welcome!!  You're making a GREAT choice! 
What about the paper work is intimidating you?  Each surgeon is different but I was required to make a list of  the diets that I've tried in the past say ten years as well as loss.  Most offices will walk you through the process-and it usually is a process when it comes to both the surgeons office and  the insurance company.
Have you spoken to the ins. co. to find out if the procedure that you want is covered in your policy?  Have you went to the informational meeting?  They  usually have a ton of info. including what  you'll need for the surgeon. They'll let you know what they need as far as records-It's really much easier than you think and the whole process goes fast as well.

I just read the posts from that other thread.  I would suggest that you have any records from the last five years handy for Dr. Buchwald (who is an amazing surgeon I might add) or his pa to look at that day.  You will prob. have an appt. either before or after the seminar.  If in doubt, call the office back and ask...they can tell you. 
Really though, try and relax hon, it's a little nerve racking but all falls into place.

for the questions about what diets you've tried and stuff, I just estimated how old I was when I did it and estimated how much weight i lost. It worked and im approved. :)

The packet says I need to bring in or have faxed in ALL of my medical records including every ct scan, xray, etc etc OR the seminar informational meeting "may" have to be rescheduled. So I have 2 weeks to get this stuff to them.

Yes, I have talked to the insurance company and the WLS DS is covered. The requirements are 6 month physician dietary plan and BMI over 40. I am over and beyond the requirements! Plus doing stuff not required of me.

awesome! Just go to your primary physician on Monday and fill out a release form for all of your medical records. I have never had a cat scan or xray so I didn't need to send any of that. The only thing in my medical records thaat ANW wanted was my weights for the last two years and my doctors recommendation, so I didn't have to worry too much about that. You are well on your way!! :)

I'm not fond of Dr. B's assistant, Danette, so don't even get me started. Just be kind to her on the phone and get your paperwork submitted and don't let her bully your around! She wanted to write my insurance appeal and I wouldn't let her. Thank God I didn't. We got it approved with the help of another "DS angel" and who the heck knows if Danette would have. She just never gave me that sense of urgency and competency, yet she must have some since Dr. B's kept her as long as he has.

Bottom line is I got approved for surgery, Dr. B. performed my surgery at the end of the calendar year to fit me in on the insurance timeline, and I am blessed beyond measure for it!

I'm game for a "pre surgery" chat anytime, so just PM me. Also, I'm usually the one to offer to meet you at the DS info seminar if you'd like. Done that plenty for potential DS'ers. Just let me know.
