Keep on them!!!  They will try to discourage you to save the expenses!!!!  Keep on track with the pre-op weight loss and appeal, appeal, appeal.  There is a person who had the approval from hell on the board named Doodlebug.  You can go into her posts, way back when she started to post and follow her journey -- talk about the appeal process.  You may get some help from those posts for your appeal.
