Crap !!! Sorry you are having problems but fate is a bad little booger!!!! Send in another packet, call them when you send it to verify the address, and also tell them that you will be calling back in 3 days later to make sure they got it -- or maybe they could call you???
It will happen -- but the tests on the way can be a pain!!!

This whole process can definitely feel like a FT job in and of itself sometimes. Hang in there girl! You WILL get through it.
One thing that helped me was to get a three-ring binder and file EVERY piece of paper related to the process so that I'd easily bring all the info with me to make phone calls at work or at home, to bring to appts, etc. I was always way more organized than the Drs offices and it helped incredibly when they needed documents in the end with insurance, etc.

Oh the memories!! I know just how you feel. I had so many hoops to jump through I didn't know what to do anymore. I just knew I had to keep fighting and I did!! Now I am 6 months out!! Don't give up...try one more time and really keep on them!!
