Approval question

I have Priority Health PPO and my Dr's office sent in a referral request and Priority Health approved me right away - my question is this - can I still be turned down by PH or MMPC if MMPC doesn't think I qualify? I have type 2 insulin and oral medication dependant diabetes and a BMI of 50. Any thoughts would be helpful.
