OK, HELP! I was denied yet again

But we knew that was coming. They cite in their letter that they did not have documentation showing that i was monitored (and compliant) for diet, exercise and behavior modification. This is so NOT the case, but unfortunately my doctors never really documented much to prove otherwise. I am writing my former doctors asking them for a letter stating that I was in fact monitored and found to be compliant while under their care on the 1800 calorie ADA diet. What Im looking for is a sample letter I can submit to my doctors that they can either insert the pertinent information into or to at least get an idea of what is needed. I did stress in my letter to them that they need to use the words monitored and compliant. This is all such b/s and HAP is a big PITA. They start out with two things in the letter that I feel are expressly and purposefully meant to discourage the patient. One is that they tell you that there is a $1,000 copay for which the patient is responsible. (Well, if the patient has done their homework, they already know this.) Secondly, they state and I quote, "...participation in a medically supervised weight loss program such as the Henry Ford WOW Program is required by surgeons prior to them performing bariatric surgery as well." Do they really think we are that stupid? Unless someone here knows differently, its my understanding that Drs. Genaw and Carlin do require their patients to lose weight, but DO NOT require them to join any specific program. They also stated they requested my medical records for review....Ummm, I sent them all of my medical records for the last three years from each doctor I have seen and they are going to tell me they requested them? WTF?
