Will this fly for the insurance company?

To Whom It May Concern:
Ms. XXXXXXX was a patient in our office between the dates of July 2002 and October 2003. At the time of her last visit, her height was 5'7" and her weight was 284 pounds giving her a body mass index of 44.4. She was being treated specifically for gestational diabetes during her pregnancies with an 1800 calorie ADA diet. While under our care, Ms. XXXXXXX was monitored and found to be compliant with her diet, exercise, and behavior modifications as required to maintain her blood sugar levels during pregnancy. She was responsible for monitoring her blood sugars at home with a glucometer and provided those results at every appointment, as well as having periodic laboratory testing for blood sugar levels. She was encouraged and counseled at all appointments on the importance of compliance and was successful with her compliance. I believe she will do well with modifications required of a bariatric surgery patient.
Sincerely,
