Do I have enough for approval??

My PCP office did not forward my bi-weekly weigh-ins (from 1/2004 to current) to the surgeons office. Tomorrow I have to drop off another records release form so they can send that information out. No biggie. The information they have is just me going in, doing weigh in and telling them what diet I was continuing to follow. No real 'consult' with the doctor every time. It is every 11 to 15 days for over two years, so is that good?? Is this going to be an issue for a denial? Also, I will be submiting a records release to the infertility clinic even though it's not within the two years time frame the insurance company is looking for (went through procedures 9-03 to 2-04). But hey, it's been determined and still an issue right?? Also, my chiropractor is going to write up a letter to send into the insurance company regarding my lower back pain and the weight being a contributing factor to the pain. I know that no one on this board can tell me the insurance company will approve, just looking for insight to make sure I have all my bases covered. Oh yeah, just found out that I am being monitored by PCP now for hypertension! Last few times it has been going up and is higher now that I have ever been before. So hypertension is now added to my co-morbidities! The doctor that did my psych exam said I do not appear to be depressed.... however, I am on Zoloft. He is going to stress in the result write up that the Zoloft is 'masking' the depression. I don't think I'm depressed, so I guess the Zoloft is working! Michelle... Sorry I didn't get a chance to return your phone call. We had just had a really busy day and my DH needed me to help him with something that night, please forgive me. As for the bi-monthly weigh-in things.. Pesonaly I would call ahead and have the doctor's office have copies of the chart/notes from those visits for you to take with you, then if needed YOU can fax them. After my insurance battle I am a FIRM believer in giving them as much if not TOO much information.. that way there are less loop holes for them to use to deny you. As for the rest of it .. it ALL sounds good to me. Im around this weekend (except for Softball and Soccer games)...feel free to ring me up. Sorry again... Erin

No reason for apologies. I went out to the hot tub and then remembered I had called you! Needless to say, I did not bring a phone out there with me! The bi-weekly weigh-ins are going down to Liv-Lite with the additional records. Gwen will include them with all the stuff to send to the insurance company. Hopefully over two years of weigh-ins will be enough. Thanks for the vote of confidence! Michelle
