Lap Band to Gastric Bypass

Curious if BCBS Michigan will pay for revision from lap-band to rouxn-y gastric bypass.  My eldest dtr had lapband implanted '06.  Lost about 45 lbs and now has gained it all back.  Has anyone had experience with BCBS of Michigan paying for revision.  She was 18 at the time and will be 21 in Feb. 2011.

My BCBS paid for my revision from lap-band to RNY.  I had lap-band in May of 2008 and had the revision in June.  It was due to documented "technical/medical" reasons.  We had two years of documentation from all my visits, fills and unfills, upper G.I.s, food and exercise journals etc...
The surgeon said that was necessary or at least made it much easier to get the revision approved.
