I feel bad you've not had any replies to your question.  I didn't have the lap-band but a friend of mine did, and didn't have to be on any restrictive diet pre op.  In fact we went out to eat together the night before surgery and she ate everything she thought she'd never be able to eat again.  I don't know how Dr. Krause works tho.
Here's hoping one or two of Dr. Krause's patients will reply to you.  Sorry I couldn't be of much help, but I didn't want you to think we were ignoring you!

Hey I don't have that dr but maybe I can help... the reason for the pre op diet is to lose enough weight to shrink your liver down which makes it easier to preform surgery and less risky plus any less abdominal fat is easier for surgery. My pre op diet is 2 weeks of nothing but protien shakes 3X a day. They said people are expected to lose 12-20 lbs (my dr lost 12 lbs and she's a very thin women to begin with when she tried the 2 week diet and she cheated).  I do believe they go off your beginning BMI before trying to lose weight from what I've heard.

Hi..

I also have BCBS of Michigan.  My BMI starting the surgery process was around 41.5 ish.  That is what is submitted to insurance.  I lost 14 lbs on my pre-op liquid diet, putting my BMI at 39.5,  and I had no problems with blue cross paying the bills.

Hope this helps,
Frances

Dr Krause did my RYN 7 years ago, call his office they should be able to tell you the diet.  I also had BCBS of MI and they paid for the entire thing.

Good luck
