BCBS PPO Insurance

Ok for awhile I was pretty sure I didn't have to have any failed weight loss documentationb ecause it said they approved you as long as it was medically neccesarry and I meet all those requirements. I've done millions of diets over the years but have no records of it.
Does anyone have any experience with this?

A lot of times if your primary care physician writes a letter to the insurance company for you stating you've tried XYZ and you ave been counselled, the insurance will take that.  That is what the Dr's in m y office have done in the past when we refer a patient for WLS.   Good Luck!!

When I was denied the first time (and only time) my doctor wrote a letter to the ins co and the denial was overturned.  I had tried a doctor supervised diet too and it didn't work.  I'd ask your pcp to write a letter stating that you have tried diets in the past, only to fail.  Hopefully he can write it convincingly.
Good luck.  If worse comes to worse you'll just have to do a 6 month doctor supervised diet.  The prize is coming.  Don't give up hope.

My SIL has that insurance  - she spoke to someone on the phone the other day and was told they dont require documentation anymore.  Good luck!!

MISA asked for my documentation as well -- I gave them about six months worth of weigh****chers weight booklets.  I don't know that BCBS PPO actually asked for them though.  I had my surgery on 12/7 and I logged into BCBS'  website I think maybe 2 weeks later everything had been submitted and paid already.

It's been five years since I had surgery so don't know if things are tougher now. I have BCBS PPO but of Delaware (although my company is in Michigan ... go figure). Anyway, I was approved with no denial first time out ... all I had to do was get a cardiac consult and a stress test. I think maybe MMPC's docs (now GHP) knew how to fill out the forms to show how many diets I had tried over the years. I didn't have documentation for any of them, but I sure was on Weigh****chers enough times.
