Insurance can be extremely frustrating trust me I know, I'm a medical biller :). If you get denied tell them you want to see in writing the policy for your group. They should have all that, or you can go to your employer, they should be able to help you too, if it's something you want them to know about.

Ultimately, stay on top of them if this is what you want.

I was denied after the first request. My doctor and I both wrote letters of appeal and I was approved in no time flat. Hope the same goes for you.

do they have a web site.

some companies have site and you can put in your group numbers and stuff from your card and you can accesses the policy that covers you and get to the information you need. If the company that provides the insurance,pays for WLS coverage, it will say and It will tell you the criteria, too

try That.

Some times you need to call several times until you get somebody with the proper answers.

Its Physician's Health Plan of Mid Michigan...they are ultimately owned by the hospital. They are looking to sell out next year (which probably doesn't bode well for how agreeable they are going to be. When i asked for their specific policy info she referred me to the one that just had some bare bones comments and not specifics..i asked about that and she referred me to the hospital. Good grief!!!

I tried to find out the requirements before I chose my ins carrier at enrollment time. I called each ins and asked about the requirements, if they'll even cover the surgerry etc... my ins was very vague so i called back and had a diifenent girl. She and i hit it off and she told me everything. I asked whar my bmi needed to be if i needed 6 months diet history, letters medical records etc... I waited 5 months for approval b/c of the Documrented diet history with monthly vsits ro my PCP, I joined curves, saw a nutrtionist and kept food journals Be patient. There is alot of waiting in the process. but oh so worth it. Barb
