You will surely want to know what your surgeons stats are.
If your surgeon is a member of OH. you can click on his name and go to his page and see his stats too.\

or try Forums/weight loss related/ failed surgery's from the top of the page.

I recommend that you read the Q & A section first so that you have an understanding of what they are talking about and you don't the the bageevas scared out of you.

Yes, there are plenty of people who WLS didn't work for.  Actual mechanical failure (when something goes wrong with the surgery) is rare and if caught by your surgeon can be fixed with additional surgeries. Sometimes you have to be loud and persistant with these types of situations of mechanical failure, but if you honestly feel something is wrong, just keep asking until you get the right answers.

More often, we hear of "failures" happening because the patient was not fully compliant with the plan.  Never dealing with the bad habits that got them to moribid obesity in the first place, not figuring out the mental part of this journey, not following the rules.... those are all things to be aware of.   There are rules we'll have to follow for the rest of our lives (protein first, taking vitamins, not drinking with meals, getting enough water, exercise).

There is a few different forums here on OH that you might want to visit.  Some will say to stay away, but I believe the more you know... the better.  You have to know ALL the risks, regrets and possible complications in order to make the most informed decision about this surgery.  It's something that affects the rest of your entire life, so you must know it all before you go under the knife.  So.... visit these forums at your own risk and only if you're sure you can read with an open mind and from the eye of education rather than fear.

Men's Forum (ok, not about the bad stuff, but it's a great resource for men to hang out without the interferance of us women-folk. I lurk here sometimes just because there are a lot of really smart men who post amazing information there.)

Memorial Boards - remember that not all these folks died because of WLS. And also watch the date of their surgery... the risks of death from WLS have greatly reduced in recently years.
