i've been denied...now I need an appeal letter.....(for PHP of mid-michigan/cofinity)
anyone know where to start in writing an appeal letter and what needs to be done?
i was denied and my insurance company wants me to see their "qualified" doctor and evualated....however they "can't" guarantee me if I can ever go back and see my own doctor (Kemmeter) - so i'm at a loss for words
I've already started to contact my doctors who managed my care last year when the acid reflux started and who took care of me after I was hospitalized with complicated pneounima w/fluid on the lung. They are going to write me letters (well two of them are, the third one is out of the country, the fourth one will only send my records and the 5th one I talk to in 3 weeks) oh and dr. kemmeter is also writing a letter
other than that any other suggestions? I have the packet coming to me and it will take 5 days....anything else I should be doing?
