Why wont BCBS MI mail me a copy of the criteria??

I called BCBS to double check on the criteria needed for weight loss surgery.  They read it off to me over the phone so I had to copy fast.  I have asked them a few times now to send me a copy of the criteria so I can use it as a check off list.  THEY WILL NOT SEND ME ONE.  Has anyone else had this issue with BCBS of Michigan?

While I guess it is not a big deal it is still frustrating and I want to make sure I cover everything when it comes to meeting the criteria so I would have liked to have had something in writing from them.

I also have bcbs I also had to write everything down.If your bmi is over 50 you are automatically covered.I believe if I remember correctly if bmi over 35 need 6 months doc supervised weight loss attempts and or two comorbidities.Try to see if they have a web site with those answers good luck to you!!!!

Keep insisting.  Speak to a manager at the call center.  If they continue to refuse, go to your employer.  It is your right to have a written copy of your insurance coverage.

I don’t know what BCBS of MI wouldn’t give you a copy of the criteria. That is bazaar. I have several copies. Lol I called back to make sure they were giving me the correct info. I asked for it to be faxed and they did. Then they would stick the original in the mail.

As a matter of fact I called BCBSM today because i too wanted to be sure i was covered and that i met the criteria but the operator said she would send it to me in the mail. she also said it may take up to 21 days to recieve it. she said if your bmi is 50 or above it should be no problem and you don't need 6 month supervised diet proof. just a letter of medical necessity from your pcp + pcyh eval.
