His answer to the referal is what I would expect. MY PCP is with Genesed and will only let me go to Hurley. My "net work" allows for McClaren and one other in Saginaw but, she will NOT let me go there.
As for the rest it sounds like he was just doing just what you were asking for in you first couple of post.
I believe you current attitude of wanting to wait and do this the right way is the best approch. Start your actual 12 month diet now and see what is happening at that time. You may even have better insurance. At that time find a PCP that is affliated with the Center you want to go to first. Than start again from there.
