I don't know where you see that it is from Massachusetts. If you follow the link and READ the title of the page it clearly states

The /ma/ in the URL is possibly for Medicare Advantage - not Massachusetts.
When I called my insurance they also stated, verbally, last week, to me that I needed 12 consecutive months of a diet history and if I missed even one month that I would have to start the process over. But then again the lady did not sound like she was having a very good day either.

I just want to throw this out there as a warning. If you decide to insure just yourself, don't go on a BC of Mi individual policy, they do not cover wls on the individual policies.
