Thanks, I have purchased the suggested book, WLS for Dummies, and it's in the mail.

I spoke with my insurance company once, and I asked for them to send me a copy of their entire policy reguarding weight loss surgery.  They told me that what they need from me is the pre-authorization from the surgeon.  I used one BLM calculator online that gave me a 50.  When I told them about the BMI I did on my own (I know it's not offical without a doctor or surgeon) they said there is no requirement for a nutritional/wloss porgram that I must do to try to lose weight (fail another diet yet again).  

I need help and advice on the insurance company....the response amounted to, we really don't have that to send out.  I believe they don't want to send it to me, but that I have a right as a consumer of my insurance to request a written copy of benefits.  My school district pays most of my premiums, but I also pay for premiums.

I believe they don't want me to know what my benefits are.

I don't know about your specific insurer, but most have the policy details available on-line.  This is how I would handle it:  Call them, say you need a copy of the policy as it pertains to WLS, and ask where you can download it from their website.  If they guide you to it, great, you have it.  If they don't have it on-line, just ask how long you can expect for it to show up in your mailbox.  Those type of direct requests aren't really questions, which gives them less opportunity to give you the run around.  Be aggressive, you have the right to the information.

Highest Known Weight  =  370#  /  59.7 bmi  @  5'6"

Current Weight  =  168#  /  26.4 bmi  :  fluctuates 5# either way  @  5'7"  /  more than 90% EWL
Normal BMI (24.9)  =  159#:  would have to compromise my muscle mass to get here without plastics, so this is not a goal.
