MMPCer's release form

I have not filled out my medical release records papers until just now!  Can you say  PROCRASTINATOR!!!  Anyways... do I sent the form to MMPC for them to send over my records to the new place?  Or do I send the form to the new place and they will take care of it???

Highest Known Weight  =  370#  /  59.7 bmi  @  5'6"

Current Weight  =  168#  /  26.4 bmi  :  fluctuates 5# either way  @  5'7"  /  more than 90% EWL
Normal BMI (24.9)  =  159#:  would have to compromise my muscle mass to get here without plastics, so this is not a goal.

It needs to go to MMPC for them to release your records to the new place.

At least I am almost positive!  I filled one out when I was at MMPC for my 1-yr appt.

Hi, You need to send it to MMPC so they can legally send your records to what ever doctor your switching to. Have a great day!
