community blue ppo insurance 6 mo diet documentation

I have community blue ppo and they require 6 mo physician supervised diet documentation.  I wonder if there is a way to get this with what I have.  I have documentation that I was under the Focused Health Solutions program which is thru our insurance and employer that has nurses calling, educating, setting goals, documentating weights and etc,.  Would this work if combined with my Dr visits.

Has anyone else had community blue and how picky are they?

The only way to know for sure is to call your insurance company and ask them. Some insurance companies will take non-physician diets (like weigh****chers) and some require that the documentation actually be a part of your medical records at the doctor's office.

It should work, as long as you have consecutive 6 months of documentation.  Especially what you weigh.  When I did mine, I went to the surgeon's office every month, and to be honest the only thing they really did was weigh me.
