BCN requirements for Roux-en-Y

Can someone please tell me what they had to do to get approval for gastric bypass surgery through Blue Care Network?  Someone told me the documented diet history changed from 12 months consecutive to 6 months consecutive.....is that true?  What are they looking for to approve you?  Do you have to join a gym, go to a nutritionist, see your primary care physician every month?  I have called my insurance company before, but they always seem to sound so grey .... I am looking for black and white.  Please tell me what you did and how you got approved for the surgery if you have BCN as an insurance carrier. Thank you for any help, Shari

It has been 2 years since my surgery so things may have changed.  If I remember right, I had to have documented weight loss attempts/dieting by my physician for 12 months - he had documented something each year for at least 5 years, I think.  I had been on a number of different diets with my weight going up and down.  I had to go to a psychologist which took all of an hour.  I had to have an ekg and chest xray.  I had to talk to a dietician a couple times and have some labwork done...  I am not sure which were required by BC and which from the surgeon.  Once my family physician sent the surgeon a letter with his recommendation on it - the surgeons office  did everything else.  I did not have to do anything with BC at all - just pay a few deductables after.  Good luck!
