Yep, put them down and like she said.. get an appt soon and talk with them about wanting wls, wanting to get healthy and seeing if they might want to help you with letter's, documention, etc.

You're going to need to get a copy of your medical chart -- and it's probably still with the old family doc. So put him down, then call to make sure they still have your chart and find out what you need to do to get a copy sent to the surgeon's office.
