Filling out forms before initial consultation and need help...

It asks for my family physician, but to be honest I don't see a doctor very often and the last time that I saw a family physician was probably 2 years ago. Do I put that physician's name even though it's been so long?

I would put that dr. down and I would make an appt with him or her soon so you can touch base with them and fill them in and possibly get there help. It can't hurt.

Yep, put them down and like she said.. get an appt soon and talk with them about wanting wls, wanting to get healthy and seeing if they might want to help you with letter's, documention, etc. Good Luck!

I couldnt agree more with the Previous posters!  I think any good bariatric surgeon would require notes from that doctor or some of kind of feedback from that physician.

You're going to need to get a copy of your medical chart -- and it's probably still with the old family doc.  So put him down, then call to make sure they still have your chart and find out what you need to do to get a copy sent to the surgeon's office. Pam

Ok, I went to the office and ordered medical records for myself, plus set an appointment up for me to speak with the doctor before I go to my initial consultation with the surgeon. Thanks everyone
