Called the number on the back of my card this morning...

and I was sent around in circles with people who didn't know what to do with me until finally I got a woman who helped me! I think she helped me anyway?! She told me that she would mail me the information for criteria of weight loss surgery, and I would receive it in 7-10 business days. I'm hoping that it has good new to show me. I am really hoping that it doesn't say I have to do a 12 month supervised diet first. Wish me luck!

Did you ask for a supervisor?  When you get the mailout- check the date and make sure it isnt from 2005....its been updated this year 2008......I am not at work but when I go in Wed I can email you the criteria-its on my work computer. Shannon

Hi Shannon, I also would like youto email me the criteria as I have the same insurance and have been following the story. When I called the insurance company they did not tell me anything about a 12 month wait time but I did not know to ask either.

It's 6 months (effective 1/1/08).  If your BMI is over 50, the 6 months is waived.
