Hi Mike Welcome!  Can you tell me what this letter of support will be used for?  Is it to submit to insurance?  Or is it that he is 'on board' with your decision for wls and that he will be there for ya??? sorry if thats lame, just wondering.

Hi and thank you!

The letter is going to be sent to the insurance for approval. Thanks for any help you can give.

The letter just needs to state your doctor recomends this surgery for you and how you would benefit from the surgery.

Well, lets see what we can do here.  I know that my surgeons office had a sample letter that my doctor typed verbatum.  EVEN WROTE HE/SHE in one spot, LOL.  I will look for my approval package and see if I can get a copy of that letter for ya.  I will PM you if I can get my hands on it!!!!! Hopefully someone else out here has one for you too.

Question: Help - I need a sample letter from PCP to surgeon or insurance company

When I went for my consultation, I did not include a letter from my PCP. Since Aetna has changed their criteria (in the middle of my trying to get approved) I'm sure they are going to want this letter.

My PCP is wonderful and told me he would do anything he could to help me get approved. I even told him I would type the letter for him - how much better could it get than that? (He's having RNY in June - so he REALLY knows what it's like).

So, if someone has a sample I could use - it would appreciate it very much.-Margaret    — Margaret Godfrey (posted on May 14, 2002)

May 14, 2002 Hi. You can click on the Your Journey link on the site to get to the footsteps page. From there you can click on the "getting authorization" footstep. That section has a sample PCP letter. Hope that helps!    — Eric Klein

I printed an article from WLS Lifestyles Magazine that tells a PCP exactly what needs to be put into a letter of support to satisfy insurance companies.  Even gives sample language.  I think I have a copy of it on my computer if you want to send me your email addy --- I can send it your way.  My PCP thanked me for making it so easy on him and he was able to get the letter done the same day as I asked him for it.  Pam

I have been treating Mr./Ms.______________ for the past _______ years.  We have discussed the risks involved with bariatric surgery and it is my professional opinion that Mr./Ms._________ is a good candidate for bariatric surgery, to reduce his/her weight.  I have counseled Mr./Ms.____________ for 12 consecutive months on a medically supervised weight loss program within the past four years with little or no results. Mr./Ms.____________ is currently being treated for hypertension, diabetes, and sleep apnea, with a history of Coronary Artery Disease.  With significant weight loss these conditions should improve.

Thank you all very much for all the information I sure with my pcp I can type the letter myself and he would have no problem signing it. Now that I have a copy to go by it should make things much easier, again thanks for everything.                     Mike
