You only need a BMI of 40 . Any co-morbidities will help ,,, odd choice of words I guess,,, but the BMI and previous attempts at weight loss will help. BCBS is pretty good about this stuff so you may be in good shape anyway. Get a letter from her in support of your surgery. Be prepared for a battle though. kp

I have BCBSM PPO and they really stuck to the 12 months documented.  I didn't have any comorbs, but they wanted to make sure that I was seeing my doc.  It wasn't consecutive, and I did have my doc write a letter stating that while I didn't always go see her just for weight loss, we did discuss it every time I saw her.  That helped get me approved. Good Luck......even if you have to do the 12 months consecutive it will fly by and then you'll be all clear.

Jen, Dr. White has the worst bedside manner ever! I spent an entire hour crying my eyes out after seeing her.  Everything she said to me regarding surgery was incorrect for my insurance.  Try not to let her get you down and call your insurance company to find out what their expectations are.  Jenn

I have BCBC MI Comm Blue PPO.....and was very fortunate to have documented many years of weight loss trials and tribulations. I wrote a letter that was about 4 pages long of my issues around all weight loss attempts and my WLS surgeon's office submitted it along with my past 2 years problems from my previous PCP to Ins and within a week or two I was approved. Is there anything your WLS surgeon's office can do to assist you in approval? If not, contact BCBS directly and see what you need to have. KP is right tho, BMI of 40 or greater is required and the co morbs will help but not all mandatory.  I had so many and my bmi was 67 something. Good luck! Stacey W

I have BCBSM PPO and did not do 12 months.  I had my PCP and neurologist write a letter for me and they waived the 12 months.  Paper work send out Oct. 17th and approved Nov. 16th.  The only co morbiities that I had were arthritis (mild) and Pseudotumor Cerebri.
