12 Month Weight Loss History
I have my first appointment at Medical Weight Loss Clinic tomorrow (so I can get the 12 months of documented weights/ and weight loss attempts) I go back and forth between 39.5 and 40.1 BMI so my question is: do I have to have a 40+BMI each time they weigh me for the entire year in order to qaulify for insurance or do I only have to have a 40+BMI when I send the preauthorization in to the insurance company?...I am w/UHC but am switching to BCBS. I have already had my first appointment at MMPC.
