Letters to Insurance company

A while back I remember quite a few people having examples of letters they sent to their insurance companies. I am in need of examples in order to play the Insurance game. I have need my PCP to submit a letter saying I was under his care for 6 months and on a diet. Also looking for examples of what the PCP letter should say in regards to the need for WLS. Lastly, looking for an example of some type of personal letter to the insurance explaining my wieght/diet history... Thanks BT
