letter of intent?

I received my packet from HF yesterday and I have to write a letter of intent? Has anyone ever had to do this? Not sure how to put it in words.I'm not good at writing my thoughts down. Any help would be greatly appreciated. thanks Dawn

Hi Dawn, I'm sending you a copy of mine as a sample to what they are looking for. I sent my letter of intent to my insurance co. after I was denied. This is what got me approved, along with a letter I wrote for my P.C.P. They put it on their letterhead and mailed it in. If you want a visitor while you're in the hospital let me know.
