PCP referral

Hello I live in Missouri but have BCBS of Mi. MY PCP has only had a few patients in the past years with BCBS and none were approved for the surgery. I was wondering if anyone had a copy of the referral from your PCP that was sent to the insurance company. I would like to share some with her so she would have a better idea of what BCBS is looking for. My diet will be complete next month. Thanks! S Goodman
