Dr. Wood in Michigan???

I am trying to get the phone number for Dr. Michael H. Wood from Michigan. He was my surgeon almost 5 years ago and I'm having a problem that I need taken care of and I need some info about my surgery....and I can't find him! I looked on here and his profile is up and people have seen him as recently as July, but his information including address and phone number are missing. HELP ME PLEASE!!!
